Beginning - This is your introduction, or beginning paragraph. You need to introduce your main character and setting (time and place), and start setting up your story.
Middle 1 - This is where the story starts getting interesting. You may want to introduce your problem here, and build on the story events that you introduced in paragraph one.
Middle 2 Choices, choices, choices.... think about your problem. In this paragraph, you are going to have your character explore two or three possible solutions to the problem they are facing. Don't make your final decision yet. Your character is going to talk about all of the possibilities, and why they might be good or bad solutions.
Middle 3 - Make a decision. Your character needs to make a final decision, explain what it is, and why it was made. 
Ending/Conclusion - Wrap it up - this is your conclusion. How does your story end? Pull everything together and make your ending make sense with the events from the story. "The End" is not a conclusion!
